As a Family Model Provider, you will open your heart and home by offering room and board to one or two individuals with disabilities. You will be responsible for providing person-centered direct personal care, including activities of daily living such as cooking, bathing, cleaning, medication administration assistance, and accompanying individuals to medical appointments. Your support will play a crucial role in helping the individuals thrive, achieve independence, and work towards their personal goals and outcomes outlined in their PCSP (Person-Centered Support Plan).

Qualifications and Experience:
- Minimum of 1 year of experience working with individuals with intellectual disabilities.
- Clear criminal background check is a must.
- Access to a safe and accessible home or apartment with a dedicated bedroom for the individuals, especially if wheelchair accessibility is required.
- Reliable vehicle and valid auto insurance and driver's license for transportation.
- Completion of all required training, including Relias, Medication Administration Certification, CPR and First Aid.
- Access to a personal computer with internet connectivity for documentation purposes.
- High school graduate or equivalent education preferred.
