What are the responsibilities and job description for the CERTIFIED PEER SUPPORTER- HOSPITAL & COMMUNITY LINKAGE-FRANKLIN COUNTY position at Thrive Peer Support LLC?
- Coordinate onsite visits to acute care settings, specifically hospital inpatient units and Emergency Departments
- Assist individuals on inpatient units after emergency department encounter navigate various levels of care as needed
- Assist peers in finding resources for harm reduction, detoxification treatment, family support, and support groups
- Work with community providers to ensure connection and adherence to to on-going care
- Work with individuals in the community once released from acute care settings to support transitional needs in the community and ensure connection to on-going care
- Responsible for maintaining accurate records and notes, confidentiality, and compiling timely reports
- Maintain records and provide assistance with appropriate documentation in files and computer database
- Provide recovery resources to individuals and their family members
- Advocate for the needs of each individual
- Create and maintain relationships with Hospital staff, Emergency Room personnel, mental health/ substance use treatment agencies and providers, and residential treatment providers
- Communicate with healthcare staff to build a rapport and coordinate care
