As a Pediatric Home Health Nurse with Thrive SPC, you will have the opportunity to work with a diverse range of patients and families, providing personalized care that meets their unique needs. Your expertise and compassion will play a critical role in helping our patients thrive at home.
Key Responsibilities:
- Provide direct patient care, including administration of medications, wound care, and vital sign monitoring
- Collaborate with multidisciplinary teams to develop and implement individualized care plans
- Educate patients and families on disease management, medication regimens, and self-care techniques
- Document patient progress and communicate effectively with healthcare providers
Requirements:
- Current licensure as an RN or LPN in the state of practice
- Current CPR certification
- 1 year of experience in pediatric home health or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
