We are looking for a dedicated and compassionate Licensed Practical/Vocational Nurse (LPN/LVN) or Registered Nurse (RN) to join our dynamic team. In this role, you will provide one-on-one care to our pediatric patients in the comfort of their own homes, promoting independence, dignity, and overall well-being.
**Responsibilities:**
* Provide direct patient care, including administering medications, wound care, and vital sign monitoring
* Develop and maintain accurate, detailed records of patient progress and treatment plans
* Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care
* Educate patients and their families on self-care techniques, medication management, and disease prevention
* Participate in ongoing education and training to enhance clinical skills and stay current with industry developments
* Perform other duties as assigned by management to support the growth and success of the organization
**Requirements:**
* Current nursing license (LPN/LVN or RN) and CPR certification
* 1 year of experience in pediatrics or home health care preferred
* Strong communication and interpersonal skills
* Ability to lift up to 50 pounds and navigate uneven terrain as needed
* Valid driver's license and reliable transportation
