Responsibilities
- Provide direct patient care to a medically fragile child in a home setting
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans
- Monitor and report changes in the child's condition to the healthcare team
Requirements
To be successful in this role, you must have:
- A current valid license to practice as a Registered Nurse or Licensed Practical Nurse issued by the State Board of Nursing
- Experience working with children with complex medical needs
