About the Role
• Develop and implement individualized care plans for each patient
• Provide direct nursing care to patients in their homes
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Educate patients and families on disease management and self-care
• Maintain accurate and detailed medical records
Requirements
• RN or LVN with current valid license to practice as issued by the State Board of Nursing
• Current CPR certification
