Job Overview
We are seeking skilled RN/LPNs to join our team as Home Health Nursing Specialists. In this role, you will provide high-quality clinical care to pediatric patients in their homes, working collaboratively with families to develop individualized care plans.
Responsibilities
- Provide 1:1 patient care to pediatric patients in their homes
- Develop and implement individualized care plans to meet the unique needs of each patient
- Work collaboratively with families to ensure seamless care transitions and communication
- Maintain accurate and detailed records of patient care and progress
