Thrive Support & Advocacy is a progressive nonprofit empowering children and adults with developmental disabilities to lead rich, active and self-directed lives. Our Family Support department provides hope, information, advocacy, and one-to-one support to parents of individuals with developmental and intellectual disabilities receiving services under DDS guidelines. The Family Support Coordinator works independently and as a part of a team to provide services and information to participants and families. This is a 40 hours/week position, with 3/4 time dedicated to Family Support Center activities and 1/4 dedicated to direct respite services with clients.

RESPONSIBILITIES

  • Design and conduct activities and groups that are of interest to the stated population.
  • Ensure that the individuals we support are treated with dignity and respect in accordance with Thrive’s Human Rights Policy.
  • Develop, implement, and monitor personalized plans of care with input from the individual and family based on the desired outcomes and personal preferences within budget.
  • Facilitate monthly, quarterly and annual contact (via phone or face-to-face) with individuals and families to discuss progress toward achieving goals identified in their support plan.
  • Ensure case management notes and progress reports are properly documented.
  • Assist individuals in Self-Advocacy, decision making, and empowerment.
  • Assist with Mass Health, Food Stamps, Social Security applications and complete all necessary documentation and paperwork required.
  • Facilitate groups and/or workshops that occur during and outside of regular business hours.
  • Serve as a liaison with other care providers and attend all necessary meetings and trainings.
  • Provide direct support to individuals in their homes and communities.
  • Case management coordination of scheduled supervision, staff meetings, evaluations, and ensuring schedules are appropriate to the needs of the individual(s).
  • Complete / verify all documentation of appointment(s) and distribute as appropriate to other providers (Dept. of Developmental Services, DDS), monitor scheduling of routine and follow up appointments (i.e., medical, dental, counselor, psych, etc.).
  • Work closely with senior staff members to ensure excellent communication and seamless collaboration.

QUALIFICATIONS:

  • Experience with administrative, organizational and management tasks.
  • Proficient analytical skills and problem-solving abilities.
  • Demonstrated ability to view challenges as opportunities and to proactively identify and implement solutions that are creative, innovative, and flexible.
  • Strong interpersonal communication (written and verbal).
  • Advanced technology skills required, including proficiency in Microsoft Office.
  • Flexible attitude to work as part of a team in a diverse workforce, promoting strategic goals and annual objectives.
  • Ability to manage several complex tasks at once while maintaining professional composure and delivering superior outcomes.

Pay: $20.00 - $22.00 per hour
