SCHEDULE:
Full-time: Monday-Friday 8a-4:30pm
SUMMARY
The Wellness Coordinator provides support and assistance to residents and staff relative to healthcare management and wellness of the program participants. General responsibilities include ensuring that program participants are attending appointments, receiving medications as prescribed, coordination and collaboration with healthcare providers (medical & psychiatric), insurance companies, pharmacies and other support services that address the overall healthcare needs of the participants. This person is also responsible for facilitating and maintaining wellness initiatives which include, but are not limited to: health-related groups and education for program participants as well as arranging staff training as needed around health-related topics. This person is also responsible for assessment of the operations of program medication rooms and audit of medication oversight processes in accordance with regulations and best practices.
ESSENTIAL DUTIES AND RESPONSIBILITIES
· Provide instruction/guidance/support/training and act as a resource to the staff of various residential program in adherence to regulations/policies, and relevant health topics.
· As needed, assist and model for staff coordination between prescribing physicians, their designees, managed care companies, labs and pharmacies to ensure all requirements for filling prescriptions are met to ensure timely supply of medication fills/refills.
· Obtain and maintain all documentation necessary to meet regulations and adhere to the Agency’s and OMH’s Medication Policies and Procedures.
· Oversee med room procedures and organization, reporting any discrepancies or problems to the Program Supervisor and/or their designee.
· Assist and at times accompanies residents to appointments to ensure communication and coordination with healthcare providers.
· Ensure accurate fulfillment of physician orders as able relative to program capability and regulatory adherence.
· Report errors and incidents in accordance to Agency Incident Management Policies and Procedures.
· Assist with the training of new staff, and re-training of staff in the Medication Policies and Procedures. Observing and supervising to prepare and give approval for staff’s readiness to perform medication monitoring duties.
· Explores changing trends with medications, ability to fill medications, coordination with physicians, managed care and pharmacies
· Identify staff training needs related to resident health and facilitate training sessions
· Identify health education needs for residents and facilitate routine educational sessions to widen resident’s understanding of health conditions and wellness opportunities.
· Ensures all documentation and entry into electronic medical record is accurate and timely.
· Other related duties as assigned.

EDUCATION and/or EXPERIENCE
Required: Possess a valid LPN license.
Preferred: A minimum of at least two years of professional experience with mental health services and experience working with individuals with cooccurring substance dependence.
Salary : $21 - $23
