Key Responsibilities
As a Care Manager, you will:
- Conduct face-to-face assessments to evaluate the social, emotional, and environmental needs of clients, determining eligibility for services within established timeframes.
- Develop and implement care plans, connecting clients with essential services while adhering to State and Federal guidelines.
- Monitor and ensure the quality of in-home and community services, taking proactive measures to address any challenges.
- Reassess clients and update care plans as required, maintaining compliance with all applicable policies.
- Document client interactions promptly and thoroughly to uphold the highest standards of care and accountability.
- Collaborate with clients, family members, caregivers, and service providers to successfully execute care plans.
- Stay informed about health and social service programs across local, State, and Federal levels.
- Participate in team-building activities, professional development, and agency-prescribed continuing education.

Qualifications
- A strong background in health and social services, with the ability to navigate complex program requirements at local, State, and Federal levels.
- Proven ability to meet deadlines, complete detailed documentation, and manage multiple responsibilities effectively.
- A commitment to servant leadership and maintaining a safe, supportive work environment.
- Must be one of the following education/work experiences: An Registered Nurse; OR Bachelor's degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health and Human Services; OR Bachelor's degree in any filed with a minimum of 2 years full time, direct service experience with the elderly or disabled; OR Masters's Degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health and Human Services; OR Associate's Degree in Nursing; OR Associate's Degree in any field with a minimum of 4 years full time, direct service experience with the elderly or disabled.

Pay: $40,000.00 - $49,900.00 per year
