What are the responsibilities and job description for the Community Based Service Provider position at Thrive2heal Services?
In this position, you will have a range of tasks that will allow you to provide services to our clients in various communities. Our Community-Based Service Provider provides Parent Mentoring, Supervised Visitation, and Therapeutic Mentoring services. Provides crisis intervention when necessary, behavioral interventions, and case coordination as needed. Develop and maintain positive working relationships with community members and referral sources, i.e., social workers, probation officers, school contacts, physicians, etc. Provide documentation, including assessments, treatment plans, progress notes, and reports. Coordinate and work within a team, monitor progress, and present concerns regarding cases. It is important that applicants have experience with children and families facing emotional and mental health issues. If you do not have this experience please do not apply.
