What We Do: Provide support services to individuals with disabilities for living successfully in the home and community.
How We Support: Our approach is person-centered and directed by the individual supported. We encourage independence, advocate for community inclusion, and support dignity of risk.
Who Do We Support: Our services are designed to meet the needs of all ages! We serve children, adolescents, young adults, adults, and seniors. There is no one too young or too old to receive our services.
When Do We Support: Our services provide supports, based on the needs of the individual. This could be a few hours once a week up to 24/7 365 days a year.
Where Do We Support: Our services are delivered wherever needed, such as family homes, individual homes, supported living sites, community locations, volunteer sites, and a variety of other locations.

Major Duties:
- Cultivate positive relationships with the individual served and others who support them
- Support individuals to learn and utilize skills for daily living
- Ensure individual rights are respected
- Complete documentation for all services delivered
- Ensure all individuals are treated with dignity and respect
- Understand and take measures to recognize and prevent abuse, neglect, and exploitation
Job Responsibilities May Include:
- Provide direct care to assigned individuals
- Assist with any needed self-care tasks, including but not limited to basic personal hygiene, toileting
- Ensure the individual’s environment is safe; assist with maintaining it, if necessary
- Remind and/or assist individuals with self-administration of medications
- Provide support for community activities and engagement
- Assist with and track all necessary PCISP and PBSP goals and strategies
- Encourage individuals for movement toward his/her wants and needs
- Assist individuals to plan meals, shop for groceries, prepare/serve meals, feed
- Help individuals with household cleaning and simple maintenance tasks
- Complete documentation for all delivered services within the identified timeframe
- Other duties, as assigned
Minimum Qualifications:
- Education: High school diploma, GED, or equivalent
- Experience/Requirements: One year of paid or volunteer experience preferred; must be 18 years old; must possess a valid, unrestricted driver’s license; clean criminal history
- Skills: Able to work independently and with others; competent oral and written communication skills; patience; compassion; adaptability; reliability; ability to work in a home-like setting; ability to complete physical aspects of job including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time.
