ThrYve Teams is an agency which recruits, trains and dispatches mentors to work with children and teens with social, emotional, mental and behavioral health challenges. A case coordinator is responsible for overseeing cases from beginning to end. A coordinator is the manager of the mentors in their caseload and responsible for the success of the families they work with.
We are looking for someone warm, compassionate and dependable with good people skills and good organization skills who is a self-starter and creative problem-solver. This is a full-time role in-person at our beautiful office in Bushwick.
A coordinator has many responsibilities including :
Seeing to it that cases assigned to you that are in the process of becoming active become active in a smooth and timely manner
Restaffing and restarting cases that have been paused in a smooth and timely manner
Keeping in regular contact with DSSs and parents / clients to see how they are doing and how the case is progressing
Striving for high utilization and high retention rates
Ensuring notes are submitted and approved in a smooth and timely manner
Resolving any issues that come up with an active case or with getting a case to active status
Helping the parent to find resources if needed
Staying organized and keeping track of all case information including notes, schedules, incidents, changes, paperwork etc
Keeping ThrYve’s data platforms up to date and accurate
