What are the responsibilities and job description for the Community Health Worker – Montmorency County position at Thunder Bay Community Health Service?
TBCHS is seeking a full-time CHW for Montmorency County.
SUMMARY: The Community Health Worker is responsible for encouraging patient and provider shared decision-making, promoting healthy patient behaviors that reduce risk, improving medication adherence, furthering patient self-management skills and linking the clinical care provided by the health Center with community-based social supports. Additionally, the CHW is responsible for reducing patient barriers to receiving care by aiding in removing the identified barriers.
HOURS OF WORK: Full time- flexible and varied. Some evening and weekend appointments may be required.
SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities.
Education/Experience/Qualifications:
- Must be 18 years of age or older.
- Must be able to travel with use of a personal vehicle.
- 2 Years Healthcare Experience Required
- BLS Required within 30 days.
Physical Demands: Medium
Travel: Travel to patient homes or community resources are required for this position. Travel to other sites may be required. Travel to training and conferences are also required.
