Description

Olympic Health & Recovery Services is seeking a Substance Use Disorder Professional to join our new Homeless Outreach Stabilization and Transition (HOST) team. This new program follows the Assertive Community Treatment model and partners with mental health, substance use disorder, and medical staff to outreach to homeless individuals in the community, camps, or shelters that are living with severe behavioral health disorders.

The ideal candidate for our Substance Use Disorder Professional is able to engage the most vulnerable individuals in a non-judgmental manner to provide clinical support, assessment, and discharge planning services.

Who are we? Thurston Mason Behavioral Health Administrative Organization (TMBH-ASO) and Olympic Health and Recovery Services (OHRS) is a quasi-governmental organization that is responsible for the network of behavioral health crisis services in Thurston and Mason counties, and is a licensed behavioral health agency with multiple programs assisting the most vulnerable population in our community through crisis services, jail services, hospital diversion, and housing.

Minimum starting salary is: $31.54 hourly negotiable with experience.

MAJOR RESPONSIBIITIES:

  • Complete brief substance use disorder assessments of each HOST participants basic needs, including behavioral health and substance use disorder condition, medical, housing, benefits, legal, safety and cultural needs, as appropriate.
  • Outreach and engage the most vulnerable individuals to assist with services, addressing behavioral and physical health needs.
  • Provide support to individuals to increase stability, assist to obtain housing, and transition individuals into long-term services for behavioral health conditions.
  • Assist the target population served with obtaining basic resources such as shelter, food, medical services, behavioral health treatment, jobs and other social and human services as needed.
  • Researches and coordinates availability of community resources to provide support and encouragement towards a path of recovery.
  • Create discharge plans for individuals exiting the program to include information about referrals made, linkages to services, as well as any follow up plans made with the individual to address their ongoing needs.
  • Maintains physical security of confidential materials and assigned equipment.
  • Demonstrates proficiency in utilization of electronic medical record system.
  • Maintains timely and accurate documentation within the electronic health record.
  • Maintains individual service records for enrolled individuals to include completed assessment, determination of eligibility, service plan that includes goals utilizing person centered approach and progress notes.
  • Provide outreach, engagement and liaison support to those people that are seen on a recurring basis which may require short term intensive case management. Facilitate services for high-need individuals across all involved systems of care.
  • Monitor contact behaviors and progress, provide assistance toward attaining pre-determined goals, and schedule appointments.
  • Promote best practices in treatment approaches, support systems, and interventions.
  • Meet with and interview contacts, families, and other care providers to assess needs and eligibility of services. Advocate needs of contacts within and outside system; liaise between contact, caregivers, and service providers. Provide client-level and system-wide troubleshooting and advocacy.
  • Follow-up with identified individuals in an effort to bridge gaps between local community, providers, and law enforcement.

SCHEDULE:  Monday-Thursday 7:30am-6:00pm

Qualifications


QUALIFICATONS:

  • Applicants must have an associate degree in human services or related field, or successful completion of 90 quarter or 60 semester college credits in courses from an approved school - per WAC 246-811-010(9) At least 45 quarter or 30 semester credits must be in courses relating to the substance use disorder profession and must include the topics listed in WAC 246-811-030. This is in accordance with the Washington State Department of Health SUDP/SUDP-T licensure requirements.
  • Washington State licensure as SUDP or SUDP-T is required.
  • Two (2) years working as an SUDP/SUDP-T is preferred.
  • Experience working with individuals who are houseless preferred.
  • Must be able to pass required background checks as required by OHRS contractual requirements.
  • Current Washington State Driver's License and proof of insurance or have requested and obtained an appropriate accommodation.  Satisfactory Motor Vehicle Record required.
  • Ability to utilize a personal computer with applicable software to successfully perform essential job functions of the position including word processing, spread sheet, analysis, information retrieval, and tracking.

 

SKILLS AND ABILITIES:

  • Familiarity and knowledge of best practices for alcohol/drug abuse treatment and prevention programs, mental health and developmental disabilities programs as well as with local social service agencies.
  • Familiarity and knowledge of Assertive Community Treatment (ACT) model. Ability to work well as a member of a team.
  • Strong written and verbal communication skills, as well the ability to work with people of diverse backgrounds and/or cultures. 
  • Ability to work in a cooperative and collaborative manner as a team member with community providers, the client, and the client’s family and support network.
  • Ability to work with general office and computer programs.  Microsoft Office and Electronic Health Records experience desired.
  • Ability to work with community providers and coordinate with varied agency partners.
  • Ability to work in high stress, high demand environment while communicating clearly and effectively
  • Ability to provide clear documentation of assessments and dispositions, and meet documentation requirements in a timely way
  • Ability to respectfully manage differences, opposing perspectives, opinions, and interests on a team.
  • Ability to remain flexible and adaptable to change.
  • Strong critical thinking skills, ability to conceptualize and implement creative solutions.
  • Demonstrates professionalism in communication with co-workers, clients, and community partners.
  • Promote and champion the values of partnership, teamwork, compassion and integrity in all dealings within and outside of OHRS.

 

WORKING CONDITIONS:

 

  • Outreach to individuals who may be living in the community, sanctioned and non-sanctioned camps, and in shelters.
  • Often performed under physically demanding, stressful, and environmentally diverse conditions, and requires the ability to adjust quickly to changing priorities and demands.
  • When in the community, does not physically interact with clients in the event of assaultive behavior, but works to maintain safety in accordance with the principles of crisis de-escalation and safety training.
  • Regular, in person, predictable attendance necessary to maintain care support to individuals.
  • Regular use of fleet vehicles and/or personal vehicle required for service in a two-county regional area.
  • Moderate physical exertion is required.  May be required to sit, stand, squat, walk short to moderate distances on uneven terrain, lift or move boxes/equipment weighing up to 25 lbs.
  • Outreach is performed in outdoor settings and subject to weather conditions.
