We are looking for a Licensed Social Worker (LBSW or LMSW) to service the Horry County area. The position is part-time (PRN), and the hours will vary based on census. Hours are not guaranteed, but will typically be 8-10 hours per week, Monday - Friday, 8am - 430pm.
Responsibilities and Functions
- Maintains the standards of Social Work excellence in accordance with the principles set forth by the South Carolina Board of Social Work, the SC Department of Health and Environmental Control, NASW, NHO, and agency policies and procedures.
- Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal-directed family care plan.
- Conducts ongoing reassessments of patient/family needs and counseling as required.
- Provides short-term crisis intervention and individual or family counseling when indicated.
- Participates as a member of the Bereavement Team as assigned.
- Observes, evaluates, and brings to team conferences information regarding psychosocial, spiritual, physicals, and financial conditions affecting the patient and family.
- Assumes the active role of advocate for the patient/family unit.
- Develops and maintains contact with appropriate agencies and services in order to promote interagency cooperation and to facilitate related referrals.
- Documents comprehensive psychosocial information clearly and concisely in a timely manner. Initial contact information recorded within 24 hours. All patient/family visits, telephone contacts, and referral actions are recorded in the Medical Record within a period of 24 hours.
- Participates in Social Services department programs as assigned by Department Direction (i.e., Crisis Intervention/School Programs, Bereavement Groups, Volunteer Training and additional new programs).
- Participates in committees, task forces, and agency events as required.
- Attends staff meetings, IDG Team, and other meetings as assigned and appropriate.
- Attends at least four in-service workshops or programs during the fiscal year.
- Participates in the orientation program as assigned.
