  • Responsible to: Program Director
  • Minimum Qualifications: High School Diploma or equivalent. One-year experience providing support to vulnerable populations. Valid driver’s license. Pass all pre-employment screenings. Current CMT License. CPR & First Aid, TB Results, MVA Driving Record, Physician letter sating ability to work with children in a residential program. 2 Years experience as a House Manager.

Position Overview

The Direct Care worker will provide direct care and supervision to individuals residing in a licensed TIME home. Staff in this position will be responsible for ensuring the safety of the individual, addressing the goals in the Individual Plan (IP) and complying with all regulatory, state and federal laws. All persons in this position must past a CJIS and CPS background check.

  • Specific Responsibilities
  • Complete all required trainings upon hire and annually.
  • Maintain required certifications and licenses.
  • Maintain active licenses (CMT, First Aid/CPR, Driver’s License)
  • Ensure the safety of all individuals in the program.
  • Prepare all meals and ensure the house is clean and chores are completed daily.
  • Meet the requirements specified in the IP.
  • Complete documentation daily and on time including but not limited to progress notes, weight, bowel and seizure chart.
  • Encourage and assist with individual choice and exploration of means to improve long-term health status.
  • Introduce and enhance community resources and membership.
  • Assist each individual in meeting the outcomes identified in the approved planning documents.
  • Participate in person-centered planning activities.
  • Provide personal care (including toileting and showering) as appropriate, for ALL people supported, not only the people to whom ‘assigned' for the shift.
  • As appropriate, use assistive technology to assist the person in gaining greater knowledge and skills as a means to enhance communication, personal independence, work productivity, etc.
  • Work respectfully and cooperatively together as a unified team member in a professional manner with other day and residential staff, families, community members
  • Maintain confidentiality of people and programs.
  • Assure compliance with all TIME Organization policies and procedures and licensing regulations.
  • Assure applicable supports are provided for each person supports.
  • Participate in person-centered planning activities.
  • Assist each individual in meeting the outcomes identified in the approved planning documents.
  • Encourage and assist with individual choice and exploration of means to improve long-term health status.
  • Completing household chores
  • Introduce and enhance community resources and membership.
  • As appropriate, use assistive technology to assist the person in gaining greater knowledge and skills as a means to enhance communication, personal independence, work productivity, etc.
  • Provide personal care (including toileting and showering) as appropriate, for ALL people supported, not only the people to whom ‘assigned' for the shift.
  • Work respectfully and cooperatively together as a unified team member in a professional manner with other day and residential staff, families, community members.
  • Assure documentation, and verbal shift reports, are completed accurately and timely before the end of each shift.
  • Identify, verbally report, complete and submit written information on any and ALL instances of potential abuse, neglect, mistreatment and/or rights allegation or concerns.
  • Maintain confidentiality of people and programs at TIME Organization.
  • Assure compliance with all TIME Organization policies and procedures and licensing regulations.
  • Order medications for residents.
  • Scheduling and providing coverage if needed.
  • Planning schedules and outings for residents.
  • Order food and plans meals.
  • Develop house cleaning schedule for staff.
  • Assist with hiring and training of new staff.
  • Participating in PCP meetings.
  • Complete quarterly progress notes.

Job Types: Full-time, Part-time, Contract

Pay: $22.00 per hour

Expected hours: 40 – 50 per week

Schedule:

  • 8 hour shift
