Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing comprehensive support and advocacy for clients, ensuring they receive the necessary services and resources to improve their well-being. This role requires a deep understanding of social work principles, crisis management, and behavioral health practices. The Case Manager will work collaboratively with clients, families, and community resources to develop effective case plans tailored to individual needs.
Responsibilities
- Conduct assessments to determine client needs and eligibility for services.
- Develop, implement, and monitor individualized case plans in collaboration with clients.
- Provide ongoing support through motivational interviewing techniques to empower clients in achieving their goals.
- Facilitate access to essential services such as child welfare programs, addiction counseling, workforce development opportunities, and Medicare resources.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
- Coordinate with multidisciplinary teams to ensure holistic care for clients.
- Supervise and mentor junior staff or interns as needed.
- Engage in crisis management interventions when necessary to ensure client safety and stability.
- Participate in training sessions and professional development opportunities to enhance skills in social work and behavioral health.
Experience
- A degree in Social Work or a related field is preferred; relevant experience may be considered in lieu of formal education.
- Proven experience in case management or a related role within child welfare or behavioral health settings.
- Familiarity with motivational interviewing techniques and crisis management strategies is highly desirable.
- Experience working with diverse populations and understanding of cultural competency in service delivery.
- Knowledge of community resources related to childcare, addiction counseling, workforce development, and Medicare is a plus.
- Strong interpersonal skills with the ability to build rapport with clients while maintaining professional boundaries.
- Excellent organizational skills with attention to detail in documentation and reporting.

Job Type: Full-time
Pay: $17.00 - $20.00 per hour
Expected hours: No more than 40 per week
