- Develop and implement individualized treatment plans based on patient needs and health status
- Maintain accurate and timely documentation of patient interactions and outcomes
- Collaborate with multidisciplinary teams to ensure comprehensive care and coordinate referrals as needed
Requirements:
- Active and unrestricted license to practice as a Family Nurse Practitioner in the state where employed
- Board certification in Family Medicine (ANCC or AANP)
- Excellent clinical and communication skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
