Position Summary
Staff pharmacists fill orders for parenteral and enteral medications, monitor patient drug therapies, and provide drug information. Staff pharmacists supervise and direct support personnel. In the absence of the pharmacy manager, a staff pharmacist may be required to assume the essential responsibilities and perform the duties of the Pharmacy Director.
Essential Functions
-Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately.
-Maintains accurate, complete patient medication profiles.
-Compounds and dispenses pharmaceuticals, including sterile, chemo-therapy and parenteral nutrition products accurately.
-Issues controlled substances to patient care areas and maintains records as required by law.
-Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
-Assists with pharmacokinetics consult service and drug dosing per organizational protocol.
-Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.
-Reads, extracts, and interprets information in patient charts accurately.
-Detects and reports suspected adverse drug reactions accurately and in a timely manner.
-Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives.
-Provides accurate, adequate and timely drug information to staff.
-Documents all clinical activities, interventions accurately and completely.
-Participates in quality improvement, medication use review activities.
-Participates in development, presentation of orientation, education, and training programs to the pharmacy, medical, nursing, other staffs.
-Supervises/directs support personnel, verifies daily activities and participates in performance appraisal of support personnel.
-Works independently with minimal supervision.
-Organizes and prioritizes work assignments.
-Ensures pharmacy services are provided in a timely manner.
-Adheres to and follows all patient experience initiatives.
-Reports to and departs from work on time and as scheduled.
-Performs other duties as assigned.
-Must be able to deal with stress while maintaining composure.
-Follows and adheres to TRMC vaccine policy(s) mandated by the Centers for Medicare & Medicaid Services (CMS).
Work Experience
-Two years of experience in a Pharmacy setting preferred.
-Health system experience preferred.
-At least 40 hours training in IV preparation and aseptic technique required.
Education
-Currently licensed to practice Pharmacy in the State of Texas required.
-Doctorate in Pharmacy or Bachelors in Science required.
