Minimum Education
- Holds an active Licensed Practical Nurse license, and is eligible to practice in the state of Kentucky.
- Currently certified in CPR.
Minimum Work Experience
Two years experience as a licensed practical nurse.Home Health scheduling experience preferred.FUNCTIONAL DEMANDS

Essential Functions
Provides direct nursing service as ordered by physician including specialized care to pediatric, adult, and geriatric patients, case management as indicated.Documents the delivery of patient care.Provides Case Management for Home and Community Based Service.Educate patient and / or caregiver on disease management and consults with the physician as needed.Reviews, educates and updates patient medications accurately and timely.ORGANIZATIONAL EXPECTATIONS
