Job Title: RN Discharge Planner
Location: Cherokee Nation W. W. Hastings Hospital 100 S Bliss Ave, Tahlequah, OK 74464
Schedule: - 4:30 PM (30-minute lunch break)
Position Summary: We are seeking a dedicated and experienced RN Discharge Planner with a background in Medical-Surgical (MS) and Intensive Care Unit (ICU) nursing. The ideal candidate will have at least 3 years of experience and will be responsible for coordinating and managing patient discharge planning activities to ensure a smooth transition from hospital to home or other care settings.
Key Responsibilities:
- Assess Patient Needs: Evaluate patients' medical, emotional, and social needs to develop individualized discharge plans.
- Develop Discharge Plans: Create comprehensive discharge plans that include post-hospital care, follow-up appointments, and necessary medical equipment or home healthcare services.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, social workers, and other healthcare professionals to ensure cohesive and effective discharge planning.
- Arrange Home Healthcare Services: Coordinate with home healthcare providers to arrange for necessary services and equipment for patients.
- Educate Patients and Families: Provide patients and their families with detailed information on discharge plans, available resources, and instructions for post-hospital care.
- Documentation: Maintain accurate and up-to-date records of discharge planning activities and patient interactions.
- Compliance: Ensure all discharge planning activities comply with hospital policies, state regulations, and accreditation standards.
Qualifications:
- Education: Registered Nurse (RN) with a valid nursing license.
- Experience: Minimum of 3 years of experience in MS/ICU nursing.
- Skills: Strong assessment, communication, and organizational skills. Ability to work collaboratively with a multidisciplinary team
