Admissions Nurse (RN)

Position Overview:

TLC Care Center is seeking a compassionate and dedicated Admissions Nurse (RN preferred) to join our team on the evening shift. As an Admissions Nurse, you will be responsible for guiding patients through the admissions process during the evening hours, ensuring timely assessments, accurate documentation, and seamless care coordination. You will play a key role in creating a positive, supportive experience for patients and their families.

Key Responsibilities:

• Conduct comprehensive nursing assessments for new admissions, reviewing medical history, current health status, and immediate care needs.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans based on patient needs and conditions.
• Ensure the completion of all required admissions documentation in compliance with TLC Care Center policies and healthcare regulations.
• Provide clear communication and education to patients and their families, answering questions and addressing concerns about the admission process.
• Coordinate with various departments, including nursing, administration, and therapy, to facilitate timely and efficient admissions.
• Monitor patients during the admission process, identifying and reporting any changes in condition to the appropriate healthcare team members.
• Maintain adherence to infection control, safety protocols, and healthcare compliance standards.
• Offer compassionate support to patients and families, helping ease the stress associated with hospital admissions.
  
  
  

Qualifications:

• Active and unrestricted Registered Nurse (RN) license in the state of [State Name].
