TLC provides comprehensive mobile case management including employment, health and wellness, education, finance management, accessing resources, and navigating the ins-and-outs of independently living. We help youth (ages 16-25) to rise up from foster care successfully by providing a safe housing environment where they can learn and practice the life skills necessary to work on self-identified life goals.Position Summary : The primary functions of the Social Worker are to uncover barriers to meeting independent living skills and goals identified in the participants youth-centered assessment plan, coordinate services, link necessary resources, and support youths ongoing daily challenges of independent living. Social Workers have a caseload of no more than 15 youth and engage in weekly contact with program participants within the participants home and community. This position requires dedication, commitment and patience in developing effective case management practices and plans while implementing policies and procedures in a teamwork model and harm reduction approach in accordance with agency policy and California Community Care Licensing Regulations, Title 22, Division 6. This position requires flexibility in personal scheduling and meeting a variety of needs as assigned on a daily basis by the immediate supervisor. Rotating on call, holiday, night and weekend hours required.
