Salary Range: $91,225.00 - $116,768.00 per year

ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES:

  • Supervises staff; directs the day-to-day work activities of the staff, conducts annual performance review, approves leave and timecards, handles employee discipline and monitors activities of the Diabetes department.
  • Coordinates work assignments as needed and identifies developmental and training needs of staff.
  • Manages Diabetes Program and Diabetes Grant-related budget, coordinates program meetings and other program activities.
  • Prepares grant proposals for diabetes management and prevention, prepares required reports and assures compliance with grant reporting requirements.
  • Oversees maintenance of diabetes registry and coordinates diabetes chart reviews.
  • Develops, implements and evaluates the Quality Assessment and Performance Improvement Program (QA/PI) for the Diabetes Program and its integration into the hospital wide QA/PI plan.
  • Coordinates and conducts the annual diabetes program audit.
  • Assumes responsibility for diabetes program policies and procedures that maintain hospital standards and accreditation status.
  • Attends and actively participates in organizational, departmental and program meetings and activities when required.
  • Assumes primary responsibility for the efficient flow of outpatient diabetes clinic, assuring smooth patient flow and progression through the various disciplines necessary for periodic monitoring of diabetes complications.
  • Performs overall screening which includes obtaining weight, blood pressure checks, vision screening, performing finger stick glucose monitor and A1C monitoring.
  • Coordinates the care of newly diagnosed patients with diabetes. Assesses the status of diabetes control and develops a plan of care. Works closely with members of the treatment team in developing the total care program for patients.
  • Schedules patients for appointments in optometry, dentistry, and other specialty clinics which adhere to the Diabetes Standards of Care.
  • Provides counseling and education to individuals, families and groups to maintain or improve their diabetes health status in both the inpatient and outpatient settings. This includes evaluating psychosocial and cultural aspects of diabetes as a chronic illness.
  • Develops and maintains an accurate roster of patients currently seen in the Diabetes Clinic, patients who are at high risk of developing diabetes, and entering newly diagnosed patients.
  • Develops a system for on-going surveillance of complications of diabetes with a built-in periodic review process, and a system for on-going diabetes screening.
  • Works to maximize resources for the population living with diabetes or at risk for diabetes, including grant writing, reporting and procurement.
  • Works in the service area, in field clinics, chapter houses, mobile units and other off-site locations as needed.
  • Participates in inpatient discharge planning, and assistance of inpatient staff in promoting comprehensive diabetes care as assigned.
  • Conduct client clinical education sessions including individual and group education in accordance with the relevant policies, procedures and lesson plans.
  • Continually studies work operations to seek improvements of clinic, outreach, and inpatient services. Reviews and analyzes records, reports and other evidence-based practices in diabetes care.
  • Participates in health fairs, conducts workshops and projects concerned with primary prevention of diabetes.
  • Participates in the development and presentation of professional workshops.
  • Participates in the planning, implementation and evaluation of diabetes related research projects. Works with Tribal programs for the purpose of promoting and understanding of the Diabetes Program.
  • Performs other duties as assigned.

MANDATORY MINIMUM QUALIFICATIONS:
Experience:
Three (3) years of Registered Nurse experience in diabetes education and/or diabetes care, and two (2) years of supervisory experience.
Education:
Bachelor of Science degree in Nursing from an accredited nursing program.
License:
Valid and unrestricted Registered Nurse license in any U.S. State or Territory.
Certificate:
Valid American Heart Association (AHA) certification in Basic Life Support (BLS) and maintain certification. Obtain Certified Diabetes Educator (CDE) certification within three (3) years from date of hire and maintain certification.
