Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will be responsible for assessing client needs, developing care plans, and ensuring access to necessary services.
Responsibilities
- Conduct comprehensive assessments of clients’ needs and circumstances.
- Develop individualized service plans that address specific goals and objectives.
- Provide crisis intervention and management support to clients in need.
- Collaborate with community resources, including healthcare providers, social services, and educational institutions.
- Monitor client progress and adjust care plans as necessary.
- Advocate for clients within the child welfare system and other relevant agencies.
- Maintain accurate records of client interactions, progress notes, and service outcomes.
Experience
Candidates should possess a background in social work or a related field with experience in the following areas:
- Childcare and child welfare systems
- Crisis management and intervention strategies
- Behavioral health practices
- Knowledge of addiction counseling techniques
- Familiarity with Medicare processes
- Supervisory experience is a plus
