What are the responsibilities and job description for the Health and Wellness Support Worker position at Top Care Support?
Top Care Support is looking to grow the support team of a young woman living independently in Brick NJ. The ideal candidates for her team will be people who are social butterflies, motivating, take initiative, have a belief in person-centered approach regarding engagement and care, and bring a sense of excitement to the day to day.
Job Responsibilities:
- Monitor the health and medical needs of individuals and immediately report any concerns, helping to track glucose levels
- Provide meal preparation and nutritional support, making this a collaborative and fun activity
- Assist with personal care, she has a lot of independence with activities of daily living but due to physical limitations will also needs support in theses areas; ranging from some physical support to reminders.
- Maintain confidentiality and privacy of all patient information compliant to HIPAA law and regulation.
- Complete daily logs and written reports.
- Assist with support at work (cute cafe in Bradley Beach)
- Increase community engagement with social and leisure activities
- Encourage engagement in physical activity, hallway walks, sit to stands, and other simple exercises designated by her Physical Therapist.
- Complete household chores, including cleaning, laundry, grocery shopping
- Errands and appointments, assisting with weekly errands for groceries, household items, personal items, etc and completing doctor appointments as scheduled.
Requirements:
- Is at least 18 years of age
- Has a passion for helping those in need
- Minimum of a High school diploma or equivalent
- Must be willing to have a background check
- Must be willing to take a drug screening
- Must be authorized to legally work in the United States
- Must have a car
- Must have valid driver's license and good driving record
- Current CPR/First Aid certification
Schedule of Services:
Weekends, every other Saturday morning 8-4 p
Saturday & Sunday 4 p-12 a
Pay rate:
$23-25/hr
