We’re hiring on behalf of our client, a nationally ranked hospital system, for three full-time RN Care Managers based onsite in Tarzana, CA.
This role is part of a high-impact Care Management team focused on care coordination, utilization review, and discharge planning. As an RN Care Manager, you will work closely with patients, families, physicians, nurses, social workers, and payers to ensure effective transitions of care and exceptional patient outcomes.
These positions offer 8-hour variable shifts, including availability for either 8:00 AM–4:30 PM or 4:30 PM–1:00 AM, with rotating weekends.
The ideal candidate will have strong clinical judgment, case management experience, and a passion for patient advocacy.

Key Responsibilities:
- Coordinate patient care across the healthcare continuum to support timely and effective transitions
- Conduct utilization management assessments and monitor patient progression
- Partner with interdisciplinary teams to develop and execute safe, individualized discharge plans
- Actively engage patients and families in care planning and decision-making
- Identify and address social determinants of health that impact recovery and long-term outcomes
- Reduce avoidable readmissions through robust post-acute planning and patient education
Qualifications:
Required:
- Bachelor’s Degree in Nursing (BSN), or currently enrolled with commitment to complete within 3 years of hire
- Active California Registered Nurse (RN) license
- Minimum 2 years of nursing experience in a healthcare setting (acute, ambulatory, or post-acute)
- At least 2 years of direct case management experience (care coordination or utilization management), OR successful completion of a Care Management Transitions in Practice (TIP) program
- California Fire and Life Safety card (required within 30 days of hire)
Preferred:
- Master’s degree in Nursing or related healthcare field
- National certification in Case Management (ACM or CCM)
