Registered Nurse (RN)

Position Overview:
We are looking for a motivated Registered Nurse to manage patient cases in a home health setting. In this role, you will travel to patients’ homes to conduct intake assessments, open and close cases, and optionally perform follow-up visits—all while setting your own schedule. Enjoy the freedom of choosing your hours and coordinating directly with your patients while being compensated on a per-patient basis. Please note that fluency in Spanish is required for this role.

Key Responsibilities:

• Patient Intake & Assessment:
• Travel to patients' homes to conduct thorough intake assessments.
• Document patient needs and open new cases in our system.
• Case Management:
• Manage patient cases from initiation through closure.
• Ensure accurate and timely documentation in compliance with agency guidelines and regulatory standards.
• Follow-Up Visits (Optional):
• Provide follow-up visits as needed to ensure continuous care.
• Receive additional compensation for optional follow-up services.
• Scheduling & Coordination:
• Independently schedule and coordinate visits directly with patients.
• Maintain a flexible work schedule that suits your lifestyle and availability.

Qualifications:

• Current Registered Nurse (RN) license.
• Fluency in Spanish is required.
• Prior nursing experience; experience in home health care is a plus but not required.
• Excellent communication and organizational skills.
• Ability to work independently with a self-motivated approach.
• Valid driver’s license and access to reliable transportation.
• Proficiency with electronic health records (preferred).
