- Base Salary: $60,000

About the Role
As a Case Manager – Medical Day Care Services, you will provide person-centered case management to individuals (age 16 ) who meet nursing facility level of care but choose to live in the community.
Key Responsibilities
- Conduct assessments using Person-Centered Planning (PCP)
- Develop and manage individualized service plans
- Coordinate Adult Medical Day Care and community-based services
- Document all service coordination activities in LTSS Maryland daily
- Conduct in-person client visits
- Monitor service effectiveness and client outcomes
- Ensure compliance with program and regulatory requirements
- Maintain accurate and timely documentation
Qualifications
Required:
- Bachelor’s degree in Human Services or related field (or approved equivalent)
- Some experience in case management or service coordination
- Strong communication, documentation, and organizational skills
- Valid driver’s license, reliable transportation, and safe driving record
- Ability to travel locally throughout Maryland
- Must be at least 21 years old
- Ability to pass background checks
- Authorized to work in the United States
Preferred:
- Experience with Adult Medical Day Care or Medicaid Waiver programs
- Familiarity with LTSS Maryland system
- Experience working with individuals with complex medical or behavioral needs
Schedule
- Full-time, Monday through Friday
