Total Care Services, Inc. (TCS) is seeking dedicated and compassionate individuals to join our team as Case Managers in our Coordination of Community Services program. At TCS, we are committed to delivering high-quality healthcare programs and services to our clients and their families. As a Case Manager, you will play a crucial role in supporting individuals with intellectual and developmental disabilities and their families in Maryland.
As a Case Manager, you will:
- Work collaboratively with clients and their families to identify needs, goals, and preferences
- Provide information about community resources and support services
- Empower clients to make informed choices and plan for their future
- Assist clients in accessing service delivery systems, resources, and supports
- Monitor and evaluate services to ensure they meet clients' needs and preferences
Work Environment: This is a hybrid position - work from your home office* with local community visits.
*Provided with a laptop, printer, and cell phone
Work Schedule: 8:30am- 5:00pm, Monday through Friday (flexible as needed)
Requirements:
We are searching for Coordination of Community Services Case Managers who are:
- Passionate about supporting our clients and their families.
- Strong advocacy for individuals with intellectual and developmental disabilities.
- Proven experience in Case Management.
- Ability to enter case notes and meet and exceed billable hours with a minimum of 7.5 hours a day into LTSS Maryland.
- Excellent written and verbal communication skills.
- Excellent organization and time management skills.
- Technologically savvy.
- Experienced Community Based Services Case Management
- Minimum of 1 to 3 years of Clinical Case Management
- Bachelor’s degree in human services field, such as psychology, social work, sociology, nursing, counseling, criminal justice, etc.
- Must be a licensed driver with reliable transportation, valid insurance, and a safe driving record.
- Must be fully vaccinated and pass all pre-employment qualifications.
