Job Description:
Requirements of the OT role:

Licensed Occupational Therapist
-
CPR/BLS Certified
-
Valid Drivers License and Auto insurance Coverage
-
Reliable transportation
-
Home health experience preferred
About the Occupational Therapist role:
-
Perform start of care and initial evaluation on patient.
-
Provides occupational therapy services to clients by assisting in assessment, treatment, program, planning and implementation, related documentation, and communication.

Identifies indications for evaluations of clients at appropriate intervals under the direction of the Director of Therapy.

Travel to a facility, or residential location to complete the patient evaluation.

Complete subsequent follow-up visits.

Conducts individual treatment, outlined in the treatment plan that includes, but is not limited to, areas of activities of daily living, functional mobility, range of motion, strength, coordination, cognitive/perceptual remediation, and functional maintenance.

Supervise COTAs/OTAs
-
Maintain a weekly caseload of 28-32 visits (full-time)

Utilize a universal electronic documentation system.

Maintains positive relationships and rapport with coworkers, supervisors, clients, family members, community personnel, and administration.
