Job Summary
Reporting to the Manager of Population Health and/or their designee, the Community Health Worker (CHW) is responsible for helping patients navigate and access services. The CHW will refer the patient as needed to case managers and other members of the health care team for follow up with social and economic issues and increase awareness and need for health prevention. This position also participates in outreach and health screening activities organized by THC’s Marketing and Business Development Department.
Contacts and interactions vary and may involve multiple constituencies such as direct interaction with THC’s executive management, community organizers, the general public, THC’s patients, physicians, colleagues, assigned staff, vendors, contractors and consultants for the purpose of providing and exchanging information.
Example of Essential Job Functions
- Establishes a supportive relationship with patients and their families to encourage their active engagement in their health care.
- Assists with scheduling appointments as needed and makes home visits as directed by supervisor.
- Collaborates with care team to help patient understand care plan and self-care management goals while trying to reduce cultural and socio-economic barriers between patients and their families and health care entities.
- Provides timely communication to the RN Coordinator/RN Team Leader or designee to report patient issues to ensure coordination of care and appropriate referrals to other members of the care team.
- Conducts eligibility determination, enrollment, and follow-up with uninsured patients.
- Assists patients with completing applications and registration forms.
- Participates in focused outreach and community health screenings and promotes services and programs at THC.
- Provides referrals for community services as appropriate and in consultation with the care team.
- Provides accurate and timely documentation and reports in compliance with THC policies and procedures.
- Assists patients in securing transportation to appointments. CHWs are prohibited from transporting patients.
- Other duties as assigned.
Minimum Education, Training and Experience Required
- High School Diploma
- Previous job experience in in health care or social services.
- Bilingual (Spanish) preferred
- Verifiable good driving record and reliable transportation
