Job Summary:
The primary purpose of this position is to plan, organize, develop, and direct all required social services to meet the needs of patients in accordance with hospital policies, regulatory requirements and community standards of practice. Social Service activities encompass accurately assessing patient/family psychosocial needs and coordinating the appropriate community resource to ensure timely care that meets compliance, quality and effectiveness standards/requirements.
The Licensed Clinical Social Worker (LCSW) conducts psychosocial assessments, determines patient and family needs, and develops a plan of treatment consistent with meeting those needs. Acts as a consultant to other disciplines. Interprets psychosocial information to patient, families and other professionals and works to link patients and their family to community support systems as needed. The LCSW actively collaborates with Senior Leadership Team to develop and maintain program excellence through education and development of interdisciplinary team members. LCSW is responsible for accreditation compliance for the social services program. Establishes clear expectations and department goals, holds self and team members accountable for meeting objectives and expectations.

SPECIFIC FUNCTIONAL RESPONSIBILITY:
- Licensed Clinical Social Worker
- Plans, develops, organizes, implements, evaluates and directs the social service programs of this hospital
- Performs consultation and/or delivery of direct psychosocial services as needed
- Conducts suicide screening assessment according to hospital policy
- Performs counseling and crisis intervention for patients/families as necessary
- Provides support and education to patients and their family members to assist in short or long term placement and referring them to the appropriate social service agencies
- Interprets the special psychosocial needs of patient/families to the Interdisciplinary Team for care planning
- Provides interventions to address events that occurs during inpatient hospital admission
- Prepares intake summaries, treatment plans, and case summaries and maintains ongoing confidential records.
- Collaborates with physicians in screening and evaluating patients for psychotropic medications, within the scope of practice
- Leads group discussions and provides individual counseling
- Social Services General Responsibilities
- Acts as a liaison between the patient, family members, the interdisciplinary team and community to assure that services to meet the patient’s needs are initiated and met in accordance with their plans of care and treatment
- Assists in the assessment, planning, arranging and implementation of psychosocial programs for patients and families
- Coordinates with local or state agencies and programs providing services to children, such as Regional Center, Child/Adult Protective Services (CPS/APS), California Children’s Services (CCS)
- Documents activities related to social services, discharge planning, community referrals, and coordination of patient services
- Demonstrates an ability to be flexible, organized, and function under stressful situations
- Collaborates with the Interdisciplinary Team to meet patient-specific, age-related needs to ensure effective resource management and quality of patient care
- Education and Staff Development
- Provides staff development and training on psychosocial assessment, reassessment, care plan development, intervention, prioritizing needs, case conferring and follow-up
- Educates interdisciplinary team members regarding patient attitudes, behavior and coping styles which may affect their approach to care
Qualifications
- Must be licensed as a clinical social worker (LCSW) by the California Board of Behavioral Sciences
- Preferred:
- Minimum of (2) years of social work experience in an acute hospital or equivalent preferred.
- Minimum of (1) year experience as pediatric social worker preferred.
- Possession of a National Provider Identifier (NPI) or obtain NPI within thirty days of hire preferred. (This is only needed if applying for CCS)
- Bilingual preferred
- Certification in Case Management or ability to achieve certification in Case Management is preferred.
- Current Basic Cardiac life Support (BCLS) certification preferred

Knowledge/Skills
- Knowledge of case management theory/practice and with current psychosocial issues within the community and healthcare environment
- Knowledge of CMS conditions of participation, CDPH state regulations, CCS, and TJC (hospital, nursing care center, and laboratory) accreditation requirements as applicable to job.
- Working knowledge of computer and software applications as applicable to job
- Possess strong communication, interpersonal and collaboration skills.
- Works responsibly in a team environment as well as independently.
- Manages confidential information effectively and appropriately.
- Must possess good work ethics and a professional image at all times.
Salary : $40 - $54
