Job Description:
Case Management Role
- Provide expert clinical guidance and support to patients and families.
- Develop and maintain effective relationships with healthcare providers and stakeholders.
Key Responsibilities:
- Patient assessments and goal-setting
- Care plan implementation and coordination
- Evaluation and revision of care plans as needed
Requirements:
- Master's degree in Nursing or related field
