We are seeking an experienced Home Health Professional to join our team as a Registered Nurse (RN). As a Home Health Professional, you will be responsible for making home visits to clients in designated geographic territories and performing assigned duties, including administration of medication, wound care, treatments, and procedures.
About the Job:
- You will follow a designated care plan in accordance with patient's needs.
- You will monitor clients' conditions and report changes to Clinical or Client Services Manager.
- You will perform client assessments as necessary.
- You will case management and coordination.
Qualifications:
- A current license as a Registered Nurse in NJ.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
- Graduation from an accredited and approved nursing program.
