Duties and Responsibilities
- Conduct thorough assessments of patients' needs and develop personalized care plans to meet those needs.
- Provide direct patient care, including medication administration, wound care, and treatments as prescribed by physicians.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
- Monitor patients' conditions and report any changes or concerns to Clinical or Client Services Manager.
Requirements
- A current license as a Registered Nurse in Vermont.
- A minimum of one year of recent, verifiable clinical experience.
- Graduation from an accredited and approved nursing program.
