About the Job
We are seeking a skilled Nurse Practitioner to fill a Palliative Care role at one of our partner facilities. As a Nurse Practitioner in this position, you will be responsible for providing high-quality, patient-centered care to individuals with serious illnesses.
Key Responsibilities
- Conduct thorough assessments of patients' medical and psychosocial needs
- Develop and implement individualized care plans in collaboration with the healthcare team
- Order and interpret diagnostic tests, prescribe medications, and provide other treatments as needed
- Educate patients and their families on disease management and self-care strategies
Requirements
- Master's degree in Nursing (MSN) or Doctoral degree in Nursing Practice (DNP)
- Current certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
- Experience working in palliative care or a related field preferred
- Strong communication and interpersonal skills
