As a Nurse Specialist, you will be an integral part of a multi-disciplinary team providing rehabilitative care and skilled nursing to clients in the comfort of their own homes. We are seeking an experienced and compassionate Registered Nurse (RN) to join our team as a Clinical Care Coordinator. Your expertise in patient care will enable you to coordinate and deliver high-quality, individualized services that meet the unique needs of each client.
About the Role:
- You will work independently and manage time effectively to ensure timely completion of tasks.
- You will have strong interpersonal skills to communicate effectively with clients, family members, and healthcare professionals.
- You will possess solid computer skills and prior experience with electronic medical records (EMR) is preferred.
- You will have a current license as a Registered Nurse in NJ and a minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- You will be able to travel to cases as assigned.
