We are seeking a compassionate and dedicated Start of Care / Admissions Registered Nurse, RN to join our team and work with our adult and senior clients on a per visit basis throughout Essex County.

To be successful in this role, you will have at least one year of nursing experience and prior Home Health and OASIS experience.

Registered Nurse (RN) Responsibilities:
- You will follow a designated care plan in accordance with patient's needs.
- You will make home visits to clients in designated geographic territories.
- You will perform assigned duties, including administration of medication, wound care, treatments, and procedures.
- You will monitor clients' conditions; reporting changes to Clinical or Client Services Manager.
- You will follow up with, execute, and properly document doctors' orders.
- You will perform client assessments as necessary.
- You will case manage and coordinate.
- You will accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
Qualifications for a Registered Nurse (RN):
- You must have a current license as a Registered Nurse in NJ.
- You must have a minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred but not required.
- You must have graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- You must be able to work independently and manage time effectively.
- You must have strong interpersonal skills.
- You must have solid computer skills; prior experience with electronic medical records (EMR) is preferred.
- You must be able to travel to cases as assigned.
