- Collaborate with healthcare teams to ensure high-quality patient care
- Educate patients and families on end-of-life care and support
- Coordinate hospice services and manage care transitions
Requirements:
- Minimum 1 year experience as an RN in a medical surgical/acute care setting
- Hospice experience preferred
- Computer proficiency, including typing and EMR systems
