A Physician Statement within the Last 12 Months is Required
PDD, Q Gold, or TSpot within 1 Year of Start is Required
Chest X-Ray within 5 Years of Start for Positive PPD is Required
Vaccines or Positive Titer are Required Annually for Positive TB Test
All Candidates Born Prior to 1957 are Exempt from Vaccination Requirement
Drug Screen within the Last 12 Months of Start is Required
