Job Summary
The Physical Therapist provides skilled therapeutic intervention and treatment to patients in a home setting, utilizing appropriate treatment plans and assessment techniques. This role involves closely monitoring patients’ progress toward their goals, ensuring compliance with physician orders, and educating patients and caregivers to prepare for discharge when appropriate.
Responsibilities
- Conduct thorough evaluations of patients' physical abilities and limitations.
- Develop and implement personalized treatment plans based on patient assessments.
- Administer therapeutic exercises, modalities, and manual therapy techniques.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families about rehabilitation processes and home exercise programs.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Maintain accurate documentation of patient evaluations, treatments, and progress notes.
- Stay current with advancements in physical therapy practices and medical terminology.
Skills
- Proficiency in Orthopedics and Outpatient therapy techniques.
- Experience in Patient care across various settings including Home health, Geriatrics, Hospital, and Pediatrics.
- Strong understanding of medical terminology related to physical therapy.
- Ability to work independently as well as part of a multidisciplinary team.
- Strong organizational skills to manage multiple patients effectively.

Qualifications:
- Indiana state licensure as a Physical Therapist.
