We are seeking a dedicated and professional Care Coordinator to join our team and help ensure our clients receive exceptional care and support.
Position Overview:
The Care Coordinator plays a critical role in ensuring that our clients receive the highest level of care by matching them with the right caregivers, monitoring their satisfaction, and maintaining smooth operations. While the role primarily focuses on client care and staff management, assisting with scheduling will only be required when the scheduling department needs additional support.
This position also requires participating in the on-call rotation after hours on some weekends. Additionally, the Care Coordinator supports recruiting, marketing, and other administrative tasks to enhance operational efficiency.
We are looking for someone with a background in home care coordination, preferably an LPN, RN, or someone with relevant experience in a similar role.
Key Responsibilities:
Client Care Coordination:
- Ensure all clients receive proper care tailored to their individual needs.
- Match clients with suitable caregivers based on skills, personality, and schedule compatibility.
- Conduct follow-ups with clients to assess satisfaction and address any concerns.
- Handle incoming client intake calls.
- Perform client intake visits and consultations.
Scheduling Support (as needed):
- Assist the scheduling department during high-demand periods or emergencies.
- Collaborate with the team to resolve last-minute scheduling challenges.
Recruiting and Training:
- Assist in recruiting and onboarding qualified caregivers.
- Ensure caregivers are properly trained and understand client-specific requirements.
Marketing and Community Engagement:
- Support marketing efforts to grow the client base, including attending community events and fostering relationships with referral sources.
- Represent Touching Hearts at Home in a professional and compassionate manner.
Administrative Duties:
- Maintain accurate records for client care plans and caregiver assignments.
- Collaborate with the leadership team to improve operational efficiency.
On-Call Support:
- Participate in the on-call rotation after hours on some weekends to address urgent client or caregiver needs.
Qualifications:
- Preferred: Licensed Practical Nurse (LPN) or Registered Nurse (RN) with experience in care coordination.
- Alternatively, significant experience in a similar role with a home care provider will be considered.
- Strong organizational and multitasking skills.
- Excellent communication and interpersonal abilities.
- Proficiency in scheduling and client management software is a plus.
- Compassionate and professional demeanor with a commitment to quality care.
