The general purpose of this position is to provide in-home remedial intervention for at-risk youth and families under the Arizona 12 Principles and in conjunction with the Child and Family Team Model.

1. Provides continual in-home and community-based direct services for at-risk youth and families.

2.    Identifies youth and family strengths and needs to enhance the Child and Family Team (CFT) planning process 

3.    Teaches a variety of skills (living, social, self-care, household management, etc.) to children, adolescents, parents, and care-givers. 

4.    Participates in the CFT by providing feedback regarding direct services delivery. 

5.    Identifies, connects, and integrates families in to a variety of community resources and outlets that build off individual and family strengths.

6.    Transports youth(s) to and from various appointments and activities as needed.

7.    Works collaboratively with all Touchstone Behavioral Health departments and employees, CFT members, and youth and families being served.   

8.    Enters clinical documentation in a timely manner into Credible (electronic medical records system) for the youth being supported.

9.    Communicates to Supervisor all updates, progress, or barriers being encountered during daily practice. 

10. Works a variety of shifts and schedules as assigned.

 

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the consumers served in the department:
