Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our team. This role is essential in providing comprehensive patient care and ensuring effective discharge planning for patients in both in our skilled nursing facility. As a Registered Nurse Case Manager, you will play a crucial role in coordinating patient care, advocating for patients’ needs, and facilitating communication among healthcare providers. Must have experience in MDS case management.
Responsibilities
- Assess and evaluate patient needs upon admission and throughout their hospital stay.
- Develop individualized care plans in collaboration with the healthcare team.
- Provide education to patients and families regarding treatment plans and post-discharge care.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Maintain accurate documentation of patient interactions and care plans in compliance.
- Collaborate with multidisciplinary teams to optimize patient outcomes and enhance the quality of care.
- Advocate for patients’ rights and needs throughout their healthcare journey.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Strong background in long term nursing.
- Knowledge of discharge planning processes and best practices.
- Excellent communication skills, both verbal and written, with the ability to effectively interact with patients, families, and healthcare professionals.
- Strong organizational skills with attention to detail.
- Ability to take rotational call.
- Ability to work independently as well as part of a collaborative healthcare team.
