What are the responsibilities and job description for the Human Services Provider II position at Town of Greenwich?
Reports to the Director of Case Management or the Director of the Human Services Programs.
Master’s degree in Behavioral Health from an accredited college or university, plus three (3) years of case management or behavioral health experience. Oral and written proficiency in Spanish is preferred.
Licenses and Certifications:
Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) in the State of Connecticut within six months of employment.
Qualifications:
Demonstrated knowledge of therapeutic counseling and casework principles and practices.
Demonstrated knowledge of Federal and State benefit and entitlement programs, as well as community resources available to individuals and families.
Demonstrated ability to make client need assessments and formulate appropriate, effective, treatment plans.
Proven ability to use clinical knowledge and experience to appropriately engage clients in life changing initiatives.
Proven ability to work independently and exercise sound judgment.
Proven ability to communicate effectively in writing and orally.
Demonstrated proficiency in the use of a personal computer and related software programs.
Proven ability to establish and maintain effective working relationships with clients, families, staff, agencies, and the public.
Valid driver’s license.
Examination: 100% Training & Experience Assessment
The examination to determine a candidate's eligibility for the position will be a Training and Experience Assessment. This type of examination requires that you completely fill out the application, adding pages for additional relevant information where needed, and attaching any documentation of certificates, degrees, other significant documents, etc. A candidate's eligibility to be interviewed will be based solely on material submitted at the time of application. Once the Eligibility List is prepared no further information may be submitted and no adjustments will be made.
Conducts client intake interviews and assesses client needs in the areas of health, employment, finances, living facilities, legal rights, and family/social relations.
Develops treatment and action plans, as well as treatment goals and outcome monitoring indicators.
Provides counseling services and implements appropriate interventions for routine and crisis situations.
Coordinates with clients, families, and outside agencies to determine eligibility for benefits and programs and assists the client with enrollment in State, Federal, Local, and private benefits programs and entitlements.
Supervises staff as assigned.
Serves as an advocate for clients in criminal, civil, housing, State, Federal, and other court proceedings.
Consults with community groups to determine appropriate referrals and to build community resources.
Utilizes the data management system to prepare and maintain case notes, assessments, and service plans.
Conducts home visits or agency visits.
Serves on community boards and committees as needed.
Performs occasional evening work to meet client and program needs.
Supports the Town's policies and philosophies.
Performs other related work as required.
All outside candidates will be required to undertake and successfully pass a pre-employment medical examination, which includes a substance abuse test that includes testing for THC (the psychoactive ingredient in marijuana), given at the Town's expense prior to employment if conditionally offered a part time, full time or safety sensitive position.
The Town of Greenwich has implemented a policy of background investigations all full and part time positions prior to hiring as part of the reference checking procedures for outside candidates.
