About the Role:
- Evaluate and implement customized treatment plans to help patients regain daily living skills.
- Use therapeutic techniques, adaptive equipment, and interventions to improve function and independence.
- Collaborate with an interdisciplinary rehab team to ensure comprehensive patient care.
- Document patient progress and maintain compliance with SNF guidelines.
- Educate patients, families, and caregivers on therapy techniques and adaptive strategies.
Key Responsibilities:
- Evaluate patients' functional abilities and develop individualized treatment plans to promote independence and self-care.
- Implement evidence-based interventions to address cognitive, physical, and sensory deficits.
- Collaborate with other healthcare professionals to coordinate care and achieve optimal patient outcomes.

Requirements:
