Role Summary

The ideal candidate will have experience working with geriatric special needs, behavioral health, and home health, as well as expertise in electronic documentation systems and cost neutrality.
A strong understanding of cultural competency and the ability to communicate effectively with members are essential.
Key Responsibilities
- Travel throughout the state to conduct face-to-face needs assessments and subsequent telephonic contact with members.
- Develop and implement individualized care plans that meet the unique needs of each member.
The successful candidate will be able to work independently, think critically, and prioritize tasks effectively while maintaining a high level of customer satisfaction.
Requirements:
- Registered Nurse in the state of DE with case management experience.
- Experience working with geriatric special needs, behavioral health, and home health.
- Strong communication and interpersonal skills.
- Ability to work flexible hours and travel regularly.
- Proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook).
