Key Responsibilities
- Conduct thorough assessments of member needs and develop personalized care plans.
- Collaborate with cross-functional teams to ensure seamless care delivery.
- Provide education and resources to members on their Social Determinants.
The successful candidate will be able to work independently, think critically, and prioritize tasks effectively while maintaining a high level of customer satisfaction.
Requirements:
- Registered Nurse in the state of DE with case management experience.
- Experience working with geriatric special needs, behavioral health, and home health.
- Strong communication and interpersonal skills.
- Ability to work flexible hours and travel regularly.
- Proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook).
